ALL persons applying for assistance must  bring the

 following information

 with you on the  first (1st) visit in order for approval.


 

  • proof of all income for each member in the household (pay stub, unemployment, retirement, SS, SSI, etc.)
  • proof of physical address ( bill etc.)
  • birth date for each member in the household
  • last 5 numbers ONLY of S.S. for each member of the household
  • fill out and sign  the application below,  page 2  fill out section 2 on;y
